My Rewards Redemption

1. This program only valid for all ayam A1 products only.

2. Customers are required to cut the BARCODE of ayam A1 products and stick on the My Rewards form. Additional form     can be used for redemption or downloaded from www.ayamA1.com.

3. Each BARCODE will be given 5 or 10 points according to types of product. (Please refer to www.ayamA1.com for     redemption details).

4. Gifts will be delivered within 4 to 6 weeks upon receiving the completed My Rewards form from the customer.

5.Incomplete and illegible forms and those without sufficient barcodes for redemption will be rejected.

6. Redemption gifts and points are subject to change without prior notice.

7. Gifts will not be delivered to P.O. Box addresses and addresses outside Malaysia. Proof of postage is not proof of     receipt.

8. This program expires on 31 December 2013 .Any forms received after the expiry date will not be entertained.
